Over the last 30 years, the International Labour Organization and Association of Oil & Gas Producers, verify that FSI accident rates in many countries have increased, since the advent of “zero harm”. Research has shown that this “zero harm” Health & Safety Management strategy does not work, has prevented learning any lessons, and led to widespread lack of transparency in health & safety incident reporting. The Key to preventing reoccurring FSI accidents is to go back in time and study the lessons learned from past accident investigations, to learn what hazards caused the accident. Our online courses examine the author, Peter Kinsey’s photographic record of historically reoccurring FSI accidents, on each work-crew, from the boneyard at kick-off to final tie-ins during a long career in pipeline construction. The resulting benefits of this training will be the prevention of harm, saving of lives and reduction, if not elimination of debilitating injuries on the projects where it is implemented.
